CLASS TITLE:  COMMUNITY HEALTH NURSE II

 

BASIC FUNCTION: 

 

Under the direction of an assigned Nursing Supervisor or Nursing Manager, operate or assist in the operation of clinics, providing general or specialized health care to the general public. 

 

DISTINGUISHING CHARACTERISTICS:

 

The Community Health Nurse II is the intermediate level classification in the Community Health Nurse series.  The Community Health Nurse II is the advanced level and operated with a high degree of independence.  The Community Health Nurse I classification is the entry level position in the Community Health Nurse series.

 

REPRESENTATIVE DUTIES:

 

Perform duties to the screening and immunization of patients, assisting with chest clinics,

providing public health information to the general public and maintaining records of

services performed.

 

Conducts child health conference clinics providing immunization of children, assess

child’s health, growth, development and nutritional status.

 

Participate in well-baby clinics to screen children; determine income status of families and

qualification for programs.

 

Conduct and participate in senior citizen preventive health clinics by providing physical

assessments, blood screening, eye exams and nutritional evaluations; provide a variety of

health information related to public health as it related to seniors.

 

Assist the attending physician with physical examinations and administer immunizations

to children.

 

Conduct and read TD and tuberculosis skin tests; conduct clinics to immunize against

influenza.

 

Instruct senior citizens of intervention methods to prevent accidents and other illnesses;

monitor seniors for prevention of early detection of problems and conditions.

 

Provide work direction and guidance to other nursing personnel and volunteers at

assigned clinics.

 

Administer skin tests, performs follow-up on tuberculosis reactor by obtaining medical

histories, taking chest x-rays, dispersing medication and maintaining charts.

 

Make home visits for the purpose of checking on infants, pre-school children and adults

with acute communicable diseases such as salmonella, shigella, hepatitis and Hanson’s

disease.

 

Provide services in compliance with the California Children Services (CCS) program in

regard to nursing intervention, health promotion and education to physically handicapped

children.

 

Provide public health information to the public regarding child and adolescent health,

disability prevention and education including how to detect disabling conditions and

teaching parent’s intervention methods to prevent and/or treat the illnesses.

 

Monitor high risk infants for prevention or early detection of problems of development and

child abuse screening.

 

Educate individuals, families and groups concerning such maters as Sudden Infant Death

Syndrome, salmonella, pregnancy and other health problems including communicable

diseases such as AIDS.

 

Conduct clinics to immunize and vaccinations as required by law; assist in providing

immunizations as required in clinic section.

 

Assist in chest clinics administering aerosols, performing eye checks and dispersing

medication.

 

Prepare State Encounter forms on individual visits and medical documentation of all

medical services rendered as required by law.

 

Maintain proper supplies for the conduct of clinic activities.

 

Provide counseling and emotional support and referral to special resources such as

Mental Health. 

 

Maintain a log of daily activities for time management purposes.

 

Attend in-service training sessions regarding current medical and nursing practices.

 

Perform related duties as assigned.

 

KNOWLEDGE AND ABILITIES:

 

KNOWLEDGE OF:

Principles and practices of nursing care.

Medicine, including but not limited to infant and child immunizations, cholera,

typhoid, rabies and flu vaccinations.

Public health, such as but not limited to, venereal disease, epidemiology,

communicable diseases, psychology and growth and development patterns

of children.

Diagnostic skills for medical evaluations.

Record-keeping techniques.

Oral and written communication skills.

Laws, rules and regulations related to assigned activities.

Policies and objectives of assigned program and activities.

Interpersonal skills using tact, patience and courtesy.

Health and safety regulations.

Technical aspects of field of specialty.

EDUCATION AND EXPERIENCE:

 

Any combination equivalent to: the requirements for licensing as a Registered Nurse, and two (2) years experience in a related environment. 

 

LICENSES AND OTHER REQUIREMENTS:

 

Valid California driver's license; licensed Registered Nurse in the State of California. 

 

WORKING CONDITIONS:

 

Clinic environment; subject to driving to conduct work.

 

Physical abilities required include standing for extended periods of time, lifting objects weighing up to 25 pounds, bending, reaching, pushing, hearing and speaking to exchange information, dexterity of hands and fingers to operate medical tools and equipment and physical strength to restrain children needing shots.

 

Incumbents may be exposed to contact with angry or abusing individuals, extreme heat, needles and blood and other bodily fluids, noisy and overcrowded clinic conditions, working around individuals with communicable diseases and guard dogs while making visits to residences.
